Cancelation/Rescheduling Policy

We realize emergencies happen and will be considered; however, our office policy is that you are responsible for 100% of the service fee for all missed and/or canceled appointments with less than 24-hour notice. In addition, we have a $50 rescheduling fee for all appointments rescheduled within 24 hours of appointment start time except for eyebrow wax and tint services. Arriving more than 15-minutes late to your appointment will be treated as a missed/no show appointment, and your credit card will be charged 100% of the service fee.

Consent and Waivers

All service post-care instructions will be different based on the service type and your unique skin and health conditions. I am aware that even with natural ingredients, there is a remote chance of an allergic reaction, and there is a possibility of an adverse reaction to the product used. I am also aware that certain services should not be performed with certain medical conditions. I also recognize there are no guaranteed results and that independent results are dependent on age, skin condition, and lifestyle, and that there is the possibility I may require further treatments of the treated areas to obtain the expected results at an additional cost.

I understand the post-treatment home care instructions list above and will receive additional post-care instructions for my unique skincare type at the time of my service. I understand how important it is to follow all instructions given to me for post-treatment care. If I have additional questions or concerns regarding my treatment or suggested home product/post-treatment care, I will immediately consult the office. Although I acknowledge that the estheticians and staff do not provide medical advice, I accept full responsibility for seeking such advice before receiving any services or products.

By clicking the box below, I hereby consent to and authorize Houston Skincare Sanctuary’s licensed estheticians to perform any of the treatments listed above. I have voluntarily elected to undergo this service or skin care treatment. Although it is impossible to list every potential risk and complication, I have been informed of possible benefits, risks, and complications. I have read and fully understand this agreement and all information detailed above. I understand the treatment and accept the risks. All of my questions have been answered to my satisfaction, and I consent to the terms of this agreement. I do not hold Houston Skincare Sanctuary’s licensed estheticians and/or staff responsible for any of my conditions that were present and not disclosed at the time of the service, which may be affected by the treatment performed, and I waive any and all claims against Houston Skincare Sanctuary and/or staff from liability and responsibility for any and all illness, injuries, damages, claims, rights and causes of action of any kind or nature, that may occur during or arising out of any services or products received on this and any future dates.

  • Facials, Peels, Microdermabrasion, Dermaplane, LED Facial, Plasma Lift

    Any facial and peel treatment we provide may consist of surface cleansing, superficial chemical peels, steam, exfoliation, application of antibacterial serums, corrective serums, and extractions. Treatments are designed to balance, hydrate, clear acne impactions, and prepare the skin for a home care regimen.

    Although rare, skin care treatments can have certain side effects such as erythema, bleeding, temporary scarring, dryness, discomfort, redness, rash, swelling, tenderness, pigmentation etc.

    Therefore, reading this document in it’s entirety and disclosing all pertinent information in the Health Questionnaire is imperative.

  • Semi-Permanent Lash Extensions and Refills

    We take every precaution to ensure your safety and wellbeing before, during and after your lash extension application. Although, please be aware of the necessary maintenance and possible risks. Lash extensions are a semi-permanent procedure. Your natural lashes will continue to grow and fall out, making touch-up or additional appointments necessary to maintain a look you desire.

    • Most clients require a "fill" appointment every 2-3 weeks.

    • Refills after 3 weeks (Max 4 weeks) are a different and higher price.

    • We do not do refills after 4 weeks, all returning lash clients after 4 weeks are considered full sets and will be charged accordingly.

    I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially envisioned due to my eye shape, quantity of my lashes, etc.

    I understand that additional conditions could occur or be discovered during the procedure, affecting my ability to tolerate the procedure. I understand some irritation, itching or burning may occur do to adhesive fumes. I understand that it is imperative that I disclose all of the information requested in the Health History Form.

    I understand the aftercare instructions and will do my part to maintain my eyelashes. I understand that many factors may affect the life of the eyelash extensions, such as water and moisture contact, weather conditions, and activities involving exposure to high temperatures.

  • Lash Lift and Tint

    We take every precaution to ensure your safety and wellbeing before, during and after your lash lift and tint application. Although, please be aware of the necessary maintenance and possible risks. Lash lift and tint are a semi-permanent procedure. Your natural lashes will continue to grow and fall out, making touch-up or additional appointments necessary to maintain a look you desire.

    I understand that while every attempt will be made to provide me with the lift and color I have chosen, my final result may not be what I initially envisioned due to the fullness of my natural eyelashes, eye shape, and quantity of my lashes, etc. There are no guarantees for length of time the lashes will stay permed.

    I understand that additional conditions could occur or be discovered during the procedure, affecting my ability to tolerate the procedure. I understand some eye irritation, itching or burning may occur do to adhesive fumes and in rare cases cause eye infections or blurriness. I understand that even though my technician perms the lashes using the proper technique, the instruments, tapes, cleaners, eye gel pads, adhesives, and removers used may irritate my eyes or require a physician’s follow-up care. I understand that it is imperative that I disclose all of the information requested in the Health History Form.

    Please be aware of the necessary maintenance and possible risks associated when safety post care is neglected: No water can come in contact with the eye area for 48 hours after the application and avoid using oil containing sunscreens, moisturizers and cleansers on lashes.

  • Waxing Services include areas on the face and body eyebrows, upper lip, nostril, armpit, arm, back, leg, bikini, and Brazilian.

    Waxing can have certain side effects such as skin removal, redness, swelling, tenderness, etc. Any waxing service we provide we take every precaution to ensure your safety and wellbeing before, during and after your face and/or body waxing service.

    Although, please be aware of the necessary maintenance and possible risks associated when safety post care is neglected; 72 hours after your waxing service please avoid peels, tanning, or wet room services, swimming/spas/hot tubs, and all home skin care protocols as recommended by my service provider.

  • Spray Tan include areas on the face and body that you would like to have a natural bronze look.

    Spray Tan can have certain side effects such as rashes, allergic reactions, dizziness etc. Any spray tan service we provide we take every precaution to ensure your safety and wellbeing before, during and after your face and/or body spray tan service.

    Although, please be aware of the necessary maintenance and possible risks associated when safety post care is neglected: 72 hours after your spray tan service please avoid peels, exfoliation, or wet room services, swimming/spas/hot tubs, and all home skin care protocols as recommended by my service provider.
